Ethics Approval
Please attach a copy of your ethics approval letter from either the university with which you are affiliated or the College of Physicians and Surgeons’ ethics committee.
Budget Summary
Please provide a detailed budget including the following information:
- List all support personnel to be used in this study (position, hours, wages, costs)
- List all services required to complete the study (lab tests, etc….)
- List all supply and equipment requirements for this study
- List any other project costs

If your application is successful, the Lupus Society of Alberta requires you to provide an annual progress report in addition to a final report on the research and findings.
